Procedure for getting the form is as follows:
Candidate are to pay for the non refundable amount of #3000 in Unimaid microfinance bank, then take the teller to the remedial units and change the teller to reciept, and collect your form..
For those that are not in Maiduguri they can send their credentials to someone they know and apply it for them..
